UnitedHealthcare, the parent company of GR, offers affordable policies to single persons, single parents, children, families, the self employed, students and the uninsured. GR offers coverage in more than 40 states, and utilizes the UnitedHealthcare network. The A.M. Best Company rating is “A” while the Standard And Poors rating is “A+.” For more than 60 years, consumers have selected these plans for their primary and secondary coverage. In addition to temporary benefits, ancillary and supplemental coverage helps fill many of the gaps that standard Marketplace or Group plans may not provide. Often, this type of plan is used if you are waiting for Medicare benefits to begin, or if COBRA and Marketplace rates are too high.

There are 10 available types of plans offered in the Keystone state. They are temporary short-term, dental, vision, accident, critical illness, disability, term life, telehealth, supplemental indemnity, and hospital indemnity. Several of these types of plans are medically-underwritten, and pre-existing conditions may not be fully or partially covered. Also, federal subsidies typically do not apply to these policies.

Temporary Coverage

Short-term coverage can be approved quickly (within one day), and provides very affordable stopgap benefits. Although semi-comprehensive options are offered, the most common policies are major medical catastrophic plans, that place a deductible on most claims. Rates are typically significantly lower than traditional individual and Group plans, although pre-existing conditions are not covered, and federal subsidies can not be utilized to lower the cost. Often, this type of plan is used if you are waiting for Medicare benefits to begin, or if COBRA and Marketplace rates are too high.

Preventative benefits are limited, and are not required to be provided on short-term plans. THE UHC Choice or Choice Plus networks can generally be utilized. Therefore, service can be provided in a state you do not reside. A referral is not required to visit a specialist, and any network physician or facility can be utilized. The following plans are offered:

Value Select – $2 million lifetime maximum benefit. The least expensive option since there are no prescription drug benefits. A discount drug card is provided. $5,000 and $10,000 maximum out-of-pocket options, and 30% and 40% coinsurance options. Deductible options range from $1,000 to $12,500. $75 Urgent Care copay. Doctor office visits must meet coinsurance and deductible.

Medical Plus – $1 million lifetime maximum benefit. Drug benefits are included with an additional $20 generic drug copay option. $5,000 coinsurance.

Medical Copay Value – $1 million lifetime maximum benefit. Two office visit copays allowed. Prescription drug coverage is optional. $10,000 coinsurance.

Medical Copay – $1 million lifetime maximum benefit. Two office visit copays allowed. Prescription drug coverage is included, along with additional RX options, including a 4-Tier drug card. $10,000 coinsurance.

Medical Plus Elite – $1.5 million lifetime maximum benefit. Drug benefits are included with an additional $20 generic drug copay option. $2,000 and $5,000 coinsurance options.

Dental Coverage

Dental Primary – The least expensive dental plan. The basic services deductible is $50. $25 copay for preventative cleanings, x-rays, oral evaluations, fluoride treatments, space maintainers, and sealants. No waiting period applies to approved preventative coverage. Basic services are subject to a 30% copay after deductible. Basic services also have a 6-month waiting period and include fillings (amalgram and composite), extractions, general and local anesthesia, and palliative treatment. Major services are not covered under this plan. The yearly benefit maximum is $1,000.

Dental Primary Preferred – The second-least expensive dental plan. The basic services deductible is $50. $25 copay for preventative cleanings, x-rays, oral evaluations, fluoride treatments, space maintainers, and sealants. No waiting period applies to approved preventative coverage. Basic services are subject to a 30% copay after deductible. Coverage includes root canals, crowns, gum disease treatment, surgical extractions, bridges, full dentures, oral surgery, inlays, onlays, and repairs to crowns, bridges, and dentures. The yearly benefit maximum is $1,000.

Dental Premier Choice – The basic services deductible is $50. $0 copay for preventative cleanings, x-rays, oral evaluations, fluoride treatments, space maintainers, and sealants. No waiting period applies to approved preventative coverage. Basic services are subject to a 20% copay after deductible. Basic services also have a 6-month waiting period and include fillings (amalgram and composite), extractions, general and local anesthesia, and palliative treatment.

Major services  have a 12-month waiting period, and are subject to 50% coinsurance and the deductible. Coverage includes root canals, crowns, gum disease treatment, surgical extractions, bridges, full dentures, oral surgery, inlays, onlays, and repairs to crowns, bridges, and dentures. The yearly benefit maximum is $1,200, but increases by $100 in each of the next three years, reaching a maximum of $1,500.

Dental Premier Elite – Similar benefits to previous plan. However, non-network provider benefits are based on reasonable and customary charge instead of network negotiated rate.

UnitedHealthcare HSA 

The HSA (Health Savings Account) is no longer offered, since UHC has withdrawn from the Pa private health insurance market. However, Group HSAs through emplyers are available.  It is possible that private plans will be offered again in 2021. This plan features lower premium payments than the Copay plans. You also have more control over how much you spend on your medical treatment and the types of procedures and tests you elect to have done.

It uses a flexible spending account feature without the “use it or lose it” provision that forces you to buy benefits you don’t need. As a result, any unused funds continue to roll over to the next year.You also get a tax deduction for depositing funds, even if the money has not yet been spent on qualified expenses.

The Bronze HSA 100 and Silver HSA 100 were previously the two featured plans in the state. The Bronze option had a $6,350 deductible while the Silver option offered a lower $3,650 deductible. There was no coinsurance on either policy so 100% of your expenses were covered once the deductible had been met.

Select Saver

Also known as a “catastrophic” plan, this type of policy was typically available to applicants under age 30, or if you received a certificate exemption for not being able to afford the required essential benefits. Like the HSA (above), this plan is currently not offered in the Keystone State.

The concept is fairly straightforward and simple. There is no coinsurance, so once the deductible is met, 100% of covered expenses are paid. Premiums are very affordable since only three primary-care physician (pcp) visits are included per year for each person. Of course, preventive benefits continue to be covered with no waiting period or applicable copay, coinsurance or deductible.

Specialist and Urgent-Care charges are subject to the deductible, although the negotiated price will save you some money. Prescriptions are handled the same way and a “Preferred Price Card” will lower your portion of the drug cost while the deductible has not been met.

The “Saver 80” plan was also ideal for early retirees wanting a policy to carry them to Medicare or anyone wanting low-cost health care coverage in exchange for paying routine medical expenses out-of-pocket. Larger  benefits were covered, including room and board, professional fees of medical personnel, intensive care unit, operating room and recovery room. Additional outpatient expense benefits were also provided such as outpatient surgery and emergency room fees for sickness and injury. The plan was discontinued, and will not be offered again.

UHC plans in Pennsylvania are offered through “Golden Rule Insurance Company.” GR is rated “A” by A.M. Best and “A+” by “Standard and Poors.” UnitedHealthCare processes the vast majority of their claims within 9-14 days and offers one of the largest US Networks with more than 580,000 physicians and care professionals and 4,900 hospitals nationwide. So if you are traveling outside of Pennsylvania, you’ll still be able to use your coverage. If you move to another state, there’s a good chance you can keep coverage since more than 25 million persons are already covered with UHC. Rates, of course, will vary, depending where you reside.
