Highmark Simply Blue is the newest of the plans and was just introduced in 2012. Primary care office visits are covered with a $35 copay (Specialists and Urgent Care at $50) without having to satisfy a deductible. All preventive visits including annual physicals and preventive-related prescriptions are covered a 100% and basic diagnostic services have only a $25 copay.


Many prescriptions (Formulary) are covered with an $8 copay on generic and $45 copay on brand. Additional specifics can be found in the brochure we attached below the article. Naturally, non-generic RX will have higher out of pocket costs to you. Emergency room visits must meet a $150 copay, which is comparable to most other Pennsylvania health insurance plans.


The two individual deductible options are $500 and $750 ($1,000 and $1,500 for a family). The maximum additional out of pocket cost is $4,000 for an individual policy and $8,000 for a family, which includes the most common coinsurance of 20%. Other benefits include a bi-annual eye exam for all covered applicants, chiropractor coverage and home health or skilled nursing facility coverage.

Highmark PPO Blue HDHP is an HSA-eligible plan that features very affordable premiums. Individual Deductible options are $1,200, $2,600 and $3,500. The family deductible options are two times the individual deductible and of course would only apply if there are two or more persons that are insured. Coinsurance after the deductible is only 10% instead of 20% on other Highmark plans.


Since the PPO Blue is a “High Deductible” plan, the deductible must be met before any non-preventive benefits are paid. However, a discount on prescriptions and the bi-annual eye exam are included. A Health Savings Account can be set up with a bank of your choice so that you can tax-deduct qualified health care, dental and vision costs.


Highmark Advance Blue features very affordable out of pocket costs. Primary care physician and specialist visits are subject to low copays of $20 and $30. Basic diagnostic services receive a low $20 copay while all preventive services are covered at 100% (as are all four Highmark plans). Prescription is fairly similar to the Simply Blue plan with some minor differences.


This plan features the identical deductible options as the PPO Blue HDHP plan although it is not an HSA-eligible plan. It is also one of Highmark’s newest plans and has become a very popular family plan.


Highmark Direct Blue offers some of the lowest-deductible plan options in Central Pennsylvania. An individual $250 or $500 ($750 and $1,500 Family deductible) deductible are the two choices. So if you have a major claim, your coverage will begin much quicker with this policy than most other medical insurance plans in Pennsylvania. The individual out of pocket (after the deductible) is also only $1,500 per person (maximum three per family).


Prescription benefits are fairly similar to the other Highmark Blue Shield plans with the exception of the $100 deductible. Preventive benefits, vision coverage and most other major medical coverages are the same as the other three plans.


So…which one is best for you? Of course, there is no right answer, but generally, if you like a copay plan, the Advance Blue plan may be your best choice. Highmark Advance Blue individual health insurance is now available in Western Pennsylvania. The Advance Blue plan is a comprehensive policy that offers comprehensive coverages at an affordable rate for both individuals and families. Covered benefits include coverage that is often found in expensive group plans. However, premiums are very low on this particular plan.

We like this plan because you do not have to meet the deductible to use the office visit and prescription benefits. The Highmark DirectBlue and PPOBlue policies place a deductible on those two coverages. Compared to many other available plans in Pennsylvania, the Advance Blue policy can potentially reduce your total out of pocket cash outlay.

Benefits

Primary care office visit expenses are covered with a low $20 copay while visits to a specialist require only a $30 copay. You do not have to meet the deductible to utilize this benefit. Also, there is no limit on the number of covered office visits per calendar year. We especially like this feature because if you are stricken with a serious illness or injury, there may be a need to visit a specialist quite frequently. And of couse, specialist charges can be quite expensive.

Preventive care expenses are covered without paying the copay or coinsurance. Once the policy is approved and coverage is in effect, you may utilize services such as annual physical exams, OBGYN exams, approved immunizations (both adult and child), mammograms and any medication that is categorized as “preventive.”

Highmark Advance Blue prescriptions are not subject to deductibles or coinsurance. Copays  range from $8 to $24 for generic drugs and $40 to $120 for brand drugs. There are also no lifetime maximum limits for these benefits. Unlike many older plans that have an annual prescription cap, this plan removes that restriction. Diagnostic expenses are subject to only a $20 copay which can be a major money-saver.

Larger medical expenses, such as inpatient and outpatient treatment, ER, normal hospital charges and ambulance fees are covered once the deductible has been met. The coinsurance is only 10% and the maximum out of pocket cost (not including the deductible) ranges from $1,000 to $3,000, depending on the chosen deductible. We always recommend selecting higher deductibles based upon the potential cost savings.

The free eye exam benefit every 24 months is an added bonus. Typically, most Pennsylvania health insurance plans do not offer this coverage. There is also some chiropractic coverage (subject to policy limitations).

Costs

Rates are calculated by your health status, age, sex, chosen deductible and the number of family members to be insured. For example, a 37 year old male would pay only $101 per month for a policy with a $1,250 deductible. Raising the deductible to $2,600 would reduce the rate to $88. A 37 year old female would pay $170 and $148 respectively.

Family premiums are based on the age of the oldest person to be insured. For example, assuming a family of five (Ages 43,40,16,14 and 10), the  $2,400 deductible policy would cost $433 while the $7,000 deductible policy would cost only $348. The family plan has ONE deductible (for everyone) and not individual deductibles. UnitedHealthcare and a few other carriers use this concept and we like it quite a bit.

Capital Blue Cross Short-Term health insurance coverage is now available. Affordable temporary medical plans for the Central Pennsylvania area are ideal if you only need benefits for six months (or less). Rates are very inexpensive and both individual and family plans can be purchased.

We provide the lowest short-term health insurance rates in Pennsylvania from the top companies such as Capital BlueCross, UnitedHealthOne, Highmark and Celtic. This type of policy will provide dependable benefits at a very affordable rate. But we also feel it’s important to properly explain the specific coverages and determine if it’s appropriate for you. Also, we represent other companies that may charge a lower rate than this plan.

The Capital Blue Cross plan is not the the cheapest temporary policy, but is one of the few plans that provides preventive benefits at 100%. Most other items are subject to a deductible, which is typical of this type of plan. An RX discount card is also provided, although the reduction in cost will only be slight.

Deductible options range from $500 to $5,000 for individuals and $1,000 to $10,000 for families. The application is short, and takes about 15-30 minutes to complete. Policies are often approved within 16-48 hours and lifetimes benefits of $1 million are available. Generally, if you have just left an employer, graduated from school or need “gap” coverage until a new policy begins, short-term coverage is advisable. The concept of major medical insurance is simple. Typically, a “deductible” must be met before any benefits are paid. Naturally, the higher the deductible, the lower the rate. After the deductible has been satisfied, all policies have a “coinsurance” that must be taken into consideration. To qualify for a policy, you must live in the 21-county service area of Central Pennsylvania and the Lehigh Valley. These counties include Fulton, Franklin, Adams, York, Lancaster, Berks, Lehigh, Northampton, Schuylkill, Northumberland, Montour, Columbia, Union, Centre, Mifflin, Juniata, Perry, Cumberland, Dauphin, Lebanon and Snyder.

The three available underwritten plans are PersonalBlue PPO, PersonalBlue HSA and Individual Short Term. Policies are underwritten quickly and physicals are rarely required.

PersonalBlue PPO   provides comprehensive coverage five deductible options ranging from $500 to $5,000. Preventive care visits are fully covered without having to meet a deductible or copay. However, other types of office visits are subject to a $30 copay AND the deductible. Emergency Room and Urgent Care visits are only subject to $100 and $50 copayments respectively.

We highly recommend adding the optional prescription rider. When added, generic, preferred brand and nonpreferred brand medications are covered. Copays range from $15 to $60 although utilizing mail-order will lower your cost. A $100 annual deductible applies for each covered person (maximum $300 per family).

PersonalBlue HSA (QHDHP) is the Pennsylvania Capital BlueCross version of a high-deductible health plan. Deductible options range from $1,500 to $5,000 with choices of 20% and 0% coinsurance. Like all Health Savings Accounts, preventive benefits are included without having to meet a deductible. And of course, the policy is considered a QHDHP (Qualified High Deductible Health Plan).

We strongly endorse the usage of HSA plans. They are a great way to lower your premiums, pay for qualified health, dental and vision expenses with tax-deductible dollars, and maintain better control over your personal health care program. We also believe selecting the 0% coinsurance is worth the small increase in premium. With this feature, other than the deductible, you pay no out-of-pocket money on covered expenses.

Individual Short-Term is designed to cover persons who need temporary medical coverage. The length of coverage runs from 30 days to 180 days and works best if you are between employers, simply uninsured, just graduated from school or waiting for a group plan to begin. Premiums are very cheap…often 50% less than conventional coverage.

The application is simplified and approval times often take less than 36 hours. Deductibles range from $3,000 to $10,000 with 20% coinsurance. Typically, pre-existing conditions are not a covered benefit. We like the Capital BlueCross short-term coverage. However, since we represent all of the large companies, usually, a lower rate is available from UnitedHealthcare or Celtic.

I have found this option to be the most popular of the three plans. It's cheap, straightforward and very easy to understand.

Pennsylvania Short Term Medical Plus Plan offers higher coverage than the “Value” plan.  Once again, coverage is available from 1-11 months. However, a deductible only applies to the term of the policy, and not for each incident. Therefore, if you have multiple major medical claims, you will only pay one deductible. After the deductible, the insurance also pays 80% of covered expenses. But you only need to pay $2,000 of out-of-pocket expenses before the policy pays 100%. It's more expensive than the "Value" plan but may include benefits you will not use.

Pennsylvania Short Term Medical Copay Plan offers more coverage and is the most expensive of the short-term options. A $35 copay applied to in-Network office visits. One visit is allowed for a 1-3-month policy, two visits for a 4-6 month policy and four visits for a 7-11 month policy.

Prescription drug coverage is also included. The generic drug copay is $15. Non-generic drugs are also covered but subject to higher copays and coinsurance. A $3,000 per person per term maximum applies. Perhaps I'm biased, but I don't often recommend this option. In my opinion, most consumers seem to feel it is not worth paying the higher cost of the "Copay" policy.

All three plans provide substantial provider discounts for many services that are subject to a deductible. UnitedHealthCare’s National Network consists of more than 650,000 health care professionals and physicians. Multiple billing and deductible options are also available providing additional flexibility. You can pay the entire cost of the policy at once or simply pay monthly (electronically through a checking account). If you need to cancel a policy before it terminates, you can request a refund on the unused portion of your coverage.

Perhaps the most popular and cost-effective option for self employed health care coverage is a Health Savings Account (HSA). An HSA is a tax-advantaged savings account that is combined with a qualified high deductible health insurance plan (HDHP). As an alternative to traditional health care, current health, dental and vision expenses, along with future qualified expenses, can be paid for on a tax-free basis.

The HDHP portion of the Pa Health Savings Account is simply a catastrophic policy that is issued by a health insurer. The plan must be “HSA-eligible” and typically will provide 80%-100% coverage once the deductible has been met. Preventive benefits such as routine annual physicals, mammograms, colonoscopies and children’s wellness visits are not subject to the deductible. Other expenses will receive a “Network negotiated discount,” that will lower the out of pocket costs. These discounts are usually 10%-25%. However, selected expenses may receive discounts up to 80%.

Self employed persons may contribute to an HSA with after-tax dollars and take the “above-line” tax deduction. However, contributions to the HSA cannot be made on a pre-tax basis and deductions cannot be taken for SECA purposes. Also, your tax filing status (separate or joint) does not affect contributions. Any contributions deposited into the account do not necessarily have to be used the year the deposit was made. You can wait quite a few years and still have access to your money.

Catastrophic health insurance plans are also available that are not HSA-eligible. Many Pa self employed persons utilize this type of coverage to keep premiums extremely low. While major medical claims are covered after the deductible, typically there is very little (or no) coverage for office visits and prescriptions. Deductible options usually range from $250 to $10,000, with $2,500 and $5,000 being the most popular choices. Preventive benefits, however, are provided without having to meet a deductible. 

If you have a pre-existing condition, you may still be able to obtain coverage. Most minor conditions do not cause a denial of coverage. A combination of serious conditions could be a reason to be concerned.
